We (refers to me, my dog(s) and all of my guests) agree to keep our dog(s) on leash at all times during their time at BOSLY’S BACKYARD, INC. We agree that my dog is sociable and have not harmed, shown aggression or threatening behavior towards any person or animal. We understand that other dogs will be present and cuts, scratches and injuries can occur.

We understand that we are solely responsible for any harm or damage caused to anyone or anything at or in the vicinity of the Ravenswood Event Center, 4011 N Ravenswood, Suite 108 Chicago, IL 60613 by myself, my guest (s) or my dog(s). 

We also understand and agree that BOSLY’S BACKYARD, INC. has relied upon my representation that my dog(s) is/are in good health, my dog(s) are up-to-date on Distemper / Parvo and Rabies vaccinations and not suffering from any contagious disease.

We hereby release and agree to hold harmless Bosly’s Backyard Inc., Kimberly Theobald, and its owners, officers, members, employees and agents from any and all liability, claims, suits, actions, loss, injuries or damage of any nature or kind, or for any liability, claims, suits, actions, loss, injury or damage which we may sustain. We agree to fully indemnify Bosly’s Backyard, Inc. and its employees or agents for any and all such liability, claims, suits, actions, losses, injury or damage which we cause. We specifically agree to indemnify and hold harmless Bosly’s Backyard Inc., Kimberly Theobald, individually and as Manager of Bosly’s Backyard Inc., and all of Bosly’s Backyard Inc.’s employees, members or agents as to any loss, claim, injury, damage or liability, sustained or incurred by an act or omission, whether negligent or intentional of an employee, representative, or agent of Bosly’s Backyard Inc.

We additionally give consent for me and my dog(s) to be photographed while at Bosly’s Backyard Inc. We agree to let Bosly’s Backyard use my dog’s image on promotional materials, which may include its website, Facebook and Instagram pages.
